Order Entered: 1. B&H shall file within 60 days of the date of this Order the information necessary to cure the filing deficiencies cited in our August 3, 2016 Order. 2. If B&H fails to file the information necessary to cure the filing deficiencies within 60 days from the date of this Order the Commission will dismiss this case by Order.

Order Entered: B&H's application as tendered on July 14, 2016, is rejected for filing until it has cured its deficiencies pursuant to 807 KAR 5:076, Sections 4(1 )(a), 4(1 )(f), 5(4)(b), 5(4)(c), 5(4)(d), 5(4)(g), and 5(4)(j). 2. B&H is granted a deviation from the filing requirements set forth in 807 KAR 5:076, Sections 4(1 )(b) and 4(1 )(c).
