Order Entered: Green Hills is granted an extension of time up to and including January 31, 2012, to file its required financial and statistical reports and gross operating reports for calendar years 2009 and 2010. Green Hills’ Motion is granted. Green Hills is assessed a civil penalty of $250.00. Payment of the penalty shall be made no later than January 31, 2011 by certified check or money order made payable to the Treasurer, Commonwealth of Kentucky, and shall be delivered to the Office of General Counsel, Public Service Commission, 211 Sower Boulevard, P.O. Box 61 5, Frankfort, Kentucky 40602-0615. 5. Green Hills’ failure to file the required reports and submit the required civil penalty within the time period specified by this Order will subject it to additional civil penalties.

Order entered: I. Green Hills Water District shall file with the Commission, no later than October 20, 201 1, its water district gross operating report for the 2009 calendar year and its water district annual report for the 2009 and 201 0 calendar years. 2. Green Hills Water District shall appear on October 27, 2011, at 1O:OO a.m., Eastern Daylight Time, in Hearing Room 1 of the Commission’s offices at 211Sower Boulevard, Frankfort, Kentucky, for the purpose of showing cause why it should not be subjected to the penalties of KRS 278.990 for its failure to comply with KRS 278.140 and 278.230(3). 3. If Green Hills Water District desires to waive its right to a hearing in this matter, it shall, no later than October 20, 2011, file the required reports and pay a penalty of $250. Payment of the penalty shall be made by certified check or money order made payable to the Treasurer, Commonwealth of Kentucky, and shall be delivered to the Office of General Counsel, Public Service Commission, 211 Sower Boulevard, P.O. Box 61 5, Frankfort, Kentucky 40602-0615.
