Final Order Entered: (1) The findings and recommendations contained in the Commission Staff's report are adopted and incorporated by reference into this Order as if fully set out herein. (2) Oldham Woods is authorized to assess a monthly fee of $36.07 for service rendered on and after the date of this Order. (3) Within 20 days of the date of this Order, Oldham Woods shall file a revised tariff sheet with the Commission setting forth the rates approved in this Order.

Order Entered: (1) Parties shall have 14 days to: a. File written comments, if any, on the findings and recommendations contained in Staff Report; and b. Request a formal hearing or an, IC with Staff. (2) A party’s failure to file objections to any finding or recommendation in Staff Report within 14 days shall be deemed as agreement with that finding or recommendation and a waiver of right to object. (3) A party’s failure to request a hearing within 14 days shall be deemed as a waiver of right to a hearing. (4) Any party that requests a hearing in this matter shall specifically state in its written request all objections to the findings and recommendations set forth in the Staff Report and shall provide a brief summary of testimony that it expects to present. (5) Within 14 days, Oldham Woods shall advise the Commission of its position on amending its requested rates to a level that will generate $46,309 annually. (6) Oldham Woods shall submit any amended request for rate adjustment to Commission with its response to the Staff Report. (7) If Oldham Woods submits an amended request for rate adjustment, it shall mail written notice to each of its customers no later than the date of filing of its amended request with the Commission. Notice shall be in the format set forth in Appendix B. (8) If the Commission does not receive any request for a hearing or for an IC with Staff within 14 days, this case shall stand submitted to the Commission for decision based upon existing record.

Order Entered: Oldham Woods shall maintain its records in such manner as will allow it, the Commission, or any customer to determine any amounts to be refunded and to whom, in the event a refund is ordered upon final resolution of this matter.
